- New Student Enrollment Application
  
  We are accepting applications through the 2028-29 school year. To apply for enrollment, please complete the form below and submit a tuition deposit using our electronic portal (or mail a check to the school office at 2112 Hollister Ave, Madison, WI 53726) for the following amount:
  $250 per child for 2026-27;
  $500 per child for 2027-28;
  $750 per child for 2028-29.
  Registration deposits will be applied to your future tuition, but are not refundable in the event your child does not attend Blessed Sacrament School (unless we are not able to offer you placement). If you are registering a child 3-6 years old, please be sure to specify if you are interested in standard 3K, 4K and 5K classroom programs or the Montessori program.

  Terms & Conditions
  
  1. Please submit $250 for EACH child registered to Blessed Sacrament School. This fee is applied to your tuition but is NOT refundable in the event your child does not attend Blessed Sacrament. IF YOU ARE APPLYING FOR FUTURE YEARS, submit $250 for EACH year you are asking us to hold the space for your child.
