- NEW Student Application
  
  This application form is for new students. An application fee is due when the form is submitted. Families will be notified in late January or early February if they have a confirmed seat for the following fall.
  (Continuing students do not need to complete this form -- continuing student re-enrollment takes place in January each year.)
  Age Requirements: Children must be 3 by September 1 to enter 3K and Montessori; 4 by September 1 to enter 4K; 5 to enter Kindergarten; and 6 to enter Gr. 1.
  Other Requirements: Children must be able to toilet independently.
  We are accepting applications through the 2028-29 school year. To apply, please complete the form below and submit the application fee using our electronic portal (families with a child already enrolled at BSS may opt to be billed via FACTS) for the following amount:
  $250 per child for 2026-27;
  $250 per child for 2027-28;
  $500 per child for 2028-29;
  $750 per child for 2029-30.
  Application fees will be applied to your future tuition, but are not refundable in the event your child does not attend Blessed Sacrament School (unless we are not able to offer you placement).

  1. Please submit $250 for EACH child's application. This fee is applied to your tuition but is NOT refundable in the event your child does not attend Blessed Sacrament. IF YOU ARE APPLYING FOR FUTURE YEARS, submit $250 for EACH year you are asking us to hold the space for your child.
